Fatal crash US 40 WB to I-80 WB

Thursday November 21, 2019

DPS Incident Notification 7594 (Crash – Fatal / Initial Information / Sent: 11-21-2019 14:07)

Section: 7 // District: A

Date: 11-21-2019

Time: 11:37

Location: US 40 WESTBOUND FLYOVER TO I-80 WB

Mile Post: 0

Travel Direction: West

Inv. Agency: UHP

Initial Description:Today at approximately 11:37 hours a 2 axle Dump Truck was traveling westbound on US 40 on the flyover to westbound I-80. The roads were wet and slick from earlier snow. The truck was loaded with large heavy landscape rocks. It is suspected that due to traveling too fast for the conditions, mixed with the weight of the truck and possibly weight shift, the solo occupant male driver lost control of the truck. The truck swerved left and made contact with the concrete barrier. The truck swerved right, made contact with the rightside barrier and partially overturned. The male driver was not wearing a seat-belt and was ejected from the truck. The male sustained fatal injuries from the ejection. US 40 westbound flyover is currently shut down and traffic is being diverted around the flyover. Lanes will reopen after investigation and cleanup is complete.

The deceased driver is Howard T Ama, age 44, of West Valley City, UT.
