Additional details on the fatal crash on U.S. 40 near MM 29

Initial Description:At approximately 10:28 hours, a white Dodge Ram pickup was traveling eastbound on US 40 near milepost 29. Multiple witnesses stated that the vehicle was traveling at a high rate of speed, above the posted speed limit. For unknown reasons, the driver of the white Dodge Ram, sharply turned into the westbound lane of travel. There was a double yellow line divider that was crossed. A large 30 foot semi box-truck was traveling westbound. The Dodge pickup truck impacted the front of the box truck at full speed and force. The driver of the Dodge Ram was killed instantly. The driver of the box truck sustained serious injuries and was flown from the scene by medical helicopter. A passenger of the box truck sustained injuries and was transported by ambulance. SR 40 is closed in both directions while an investigation is being completed. A small Hazmat spill is also being attended to.

The deceased driver is Steven H Okelberry, age 59, of West Valley City.
